पहले देसी कोरोना वैक्सीन कोवैक्सीन के साथ-साथ ऑक्सफर्ड यूनिवर्सिटी और एस्ट्राजेनेका के वैक्सीन कोविशील्ड को भारत में सीमित उपयोग की मंजूरी पर कांग्रेस पार्टी के एक धड़े की तरफ से सवाल तो उठाए ही गए हैं, कुछ विशेषज्ञ भी मंजूरी की प्रक्रिया को अपारदर्शी बताते हुए दोनों टीकों के क्लीनिकल ट्रायल के डेटा सार्वजनिक करने की मांग कर रहे हैं। इनका कहना है कि भारत बायोटेक और एस्ट्राजेनेका को क्रमशः कोवैक्सीन और कोवीशील्ड के क्लिनिकल ट्रायल में प्राप्त डेटा को सार्वजनिक किया जाना चाहिए ताकि उनका स्वतंत्र आकलन किया जा सके। इससे किसी भी खतरे की आशंका को लेकर दूध का दूध और पानी का पानी हो सकेगा।
मंजूरी देने की प्रक्रिया पर भरोसा पैदा करना जरूरी
इंटरनैशनल असोसिएशन ऑफ बायोएथिक्स के पूर्व अध्यक्ष डॉ. अनंत भान ने कहा कि भारत के सिवा सिर्फ रूस और चीन ने ही अपने-अपने वैक्सीन का एफिकेसी डेटा सार्वजनिक किए बिना ही उपयोग की मंजूरी दे दी। उन्होंने कहा, “नियामकीय प्रक्रिया में भरोसा पैदा करना महत्वपूर्ण है, लेकिन यहां तो कई चिकित्सक भ्रम की स्थिति में हैं और पूछ रहे हैं कि आखिर कौन सा वैक्सीन काम करेगा। जिस तरह से मंजूरी दी गई है उस पर विश्वास नहीं किया जा सकता है और जिस तरह की भाषा का प्रयोग किया गया है वो रचनात्मक लेखन लगता है न कि कानून पर आधारित।”
द टाइम्स ऑफ इंडिया में प्रकाशित एक लेख में इसे लेकर विस्तार से जानकारियां दी गई हैं। ‘A Vaccine-Credibility Problem?’ नाम से प्रकाशित इस लेख में दिव्या राजगोपाल लिखती हैं, “दुनियाभार में किसी वैक्सीन को मंजूरी देने से पहले तीन कारकों पर विचार किया जाता है- सुरक्षा, रोग प्रतिरोधक क्षमता पैदा करने की ताकत और प्रभावोत्पादकता।”
वो आगे कहती हैं कि सामान्य परिस्थितियों में किसी वैक्सीन कैंडिडेट के क्लिनिकल ट्रायल की प्रक्रिया पूरा करने में सालों लग जाते हैं, लेकिन विश्व स्वास्थ्य संगठन के नियमों के तहत अगर प्रभावोत्पादतकता (एफिकेसी) के न्यूनतम सुनिश्चित मापदंड पूरे कर लिए जाएं तो कुछ प्रक्रियाओं के पूरा होने से पहले वैक्सीन के उपयोग की मंजूरी दी जा सकती है। कोविड-19 के मामले में न्यूनतम प्रभावोत्पादकता की अनिवार्यता 50% से ज्यादा सुनिश्चित किया गया है। प्रभावोत्पादकता का मतलब प्रभावशीलता नहीं होता है बल्कि यह वो आंकड़ा होता है जो बताता है कि टीका लगाए जाने के बाद बीमारी कितने प्रतिशत की कमी आई।
भारत बायोटेक के मुताबिक कोवैक्सीन के तीसरे चरण का क्लिनिकल ट्रायल भी पूरा नहीं हुआ है लेकिन जारी है। लेख में तमिलनाडु में वेलोर स्थित क्रिश्चियन मेडिकल कॉलेज के डिपार्टमेंट ऑफ गेस्ट्रोइंटेस्टिनल साइसेंज में प्रफेसर गगनदीप कांग के हवाले से कहा गया है कि वैक्सीन कैंडिडेट के तीसरे चरण के क्लीनिकल ट्रायल में कम-से-कम छह हफ्ते का वक्त लगता है। उसके बाद ही वैक्सीन का एफिकेसी के अंतरिम आंकड़े आ पाते हैं।
केंद्र सरकार कहती है कि टीकाकरण के अधिक विकल्पों के लिए क्लिनिकल ट्रायल मोड में काफी एहतियात बरतने के बाद ही भारत बायोटेक के वैक्सीन को आपातकालीन परिस्थिति में सीमित उपयोग की मंजूरी दी गई है। लेख में दावा किया गया है कि दरअसल भारत बायोटेक के वैक्सीन के सुरक्षा मानकों पर खरे उतरने के कारण सीमित उपयोग को मंजूरी दे दी गई, उसका एफिकेसी डेटा का इंतजार नहीं किया गया।
विश्व स्वास्थ्य संगठन के निर्धारित नियम में एक प्रावधान है जो ब्रिटेन और अमेरिका की नियामक संस्थाओं की तरफ से किसी वैक्सीन को उपयोग की मंजूरी मिल जाने के बाद दूसरे देशों के रेग्युलेटरों को भी अपने यहां उपयोग की अनुमति देने का अधिकार देता है। भारत बायोटेक ने कहा है कि वह अपने वैक्सीन का देश में सबसे बड़ा क्लीनिकल ट्रायल कर रही है जिसमें 23 हजार लोग भाग ले रहे हैं। हालांकि, उसने 100 लोगों का एफिकेसी डेटा साझा नहीं किया जो वैक्सीन की मंजूरी के लिए न्यूनतम पैमाना है।
चीन और रूस समेत दुनियाभर की दवा निर्माता कंपनियों ने अपने-अपने वैक्सीन कैंडिडेट की सेफ्टी और एफिकेसी के डीटेल विभिन्न पत्रिकाओं में प्रकाशित किए। अमेरिकी नियामक संस्था खाद्य एवं दवा प्रशासन आमसभा बुलाती है जहां वैक्सीन के लिए मंजूरी मांगने वाली कंपनी को विशेषज्ञ समूह के सामने आंकड़े पेश करने होते हैं। इस आमसभा में क्या हुआ, क्या नहीं, यह इंटरनेट पर उपलब्ध होता है। वहीं, ब्रिटेन की रेग्युलेटरी बॉडी दवा और स्वास्थ्य देखभाल उत्पाद नियामक एजेंसी ने ऑक्सफोर्ड-एस्ट्राजेनेका के वैक्सीन कोवीशील्ड को उपयोगी की अनुमति देने पर एक प्रेस कॉन्फ्रेंस किया था। भारत बायोटेक के मामले में भी ऐसा ही हुआ, लेकिन संवाददाताओं को सवाल नहीं पूछने दिए गए।
‘क्लिनिकल ट्रायल के बीच उपयोग की मंजूरी हैरान करने वाला फैसला’
उधर फिजिशियन, रिसर्चर और स्टैफर्डशायर यूनिवर्सिटी में विजिटिंग प्रफेसर डॉ. जेएन राव ने कहा, “हमें भारत बायोटेक के तीसरे चरण के कम-से-कम शुरुआती परिणाम आने तक इंतजार करना चाहिए थे। हां, महामारी है और हमें वैक्सीन भी चाहिए लेकिन यह भी बहुत जरूरी है कि आम जनता का वैक्सीन में भरपूर भरोसा हो। कोई वैक्सीन सिर्फ इसलिए भरोसेमंद नहीं हो जाता है कि उसे मंजूरी मिल गई है। किसी वैक्सीन को मंजूरी तभी दी जाती है जब उसने क्लिनिकल ट्रायल्स में अपनी विश्वसनीयता साबित की हो।”

भारतीय औषधि महानियंत्रक के प्रेस कॉन्फ्रेंस में जिन जानकारियों के सामने आने की अपेक्षा की गई थी वो नहीं दी गईं और सवाल पूछने की भी अनुमति नहीं दी गई। ऑल इंडिया ड्रग ऐक्शन नेटवर्क की मालिनी आइसोला ने कहा, “दोनों कोरोना वैक्सीन को मंजूरी देने के मामले में यह नहीं बताया गया है कि डीजीसीआई ने कानून के किस विशेष प्रावधान के तहत सीमित आपातकालीन उपयोग की मंजूरी दी है। साथ ही, किन शर्तों पर यह मंजूरी दी गई, इसका भी कोई जिक्र नहीं है।” AIDAN ने बयान जारी कर डीजीसीआई से उन सभी आंकड़ों और विश्लेषणों को सार्वजनिक करने की मांग की है जिनके आधार पर कोवैक्सीन और कोवीशील्ड को सीमित उपयोग की मंजूरी दी गई है। संस्था का कहना है कि डेटा सार्वजनिक हो जाने पर उसकी स्वतंत्र रूप से पुष्टि हो की जा सकती है।
अमेरिका और ब्रिटेन की तरह उपलब्ध कराए जाएं दस्तावेज
AIDAN के साथ-साथ देश की प्रतिष्ठित वैक्सीन एक्सपर्ट डॉ. गगनदीप कांग ने भी इस दावे पर सवाल उठाया है कि कोवैक्सीन ब्रिटेन में पाए गए कोरोना वायरस के नए स्ट्रेन के खिलाफ भी कारगर साबित होगा। उन्होंने कहा कि इस दावे की पुष्टि के लिए कोई वैज्ञानिक आंकड़े नहीं दिए गए हैं। महामारी विशेषज्ञ डॉ. गिरिधर बाबू ने सवाल किया कि जिन पैमानों पर वैक्सीन को मंजूरी दी गई, उससे संबंधित दस्तावेज मुहैया क्यों नहीं कराए गए जैसा कि अमेरिका और ब्रिटेन की नियामकीय संस्थाओं ने किया। वहीं, इंडियन जर्नल ऑप मेडिकल एथिक्स के संपादक डॉ. अमर जिशानी ने पूछा, “किन लोकल एफिकेसी डेटा के आधार पर एक्सपर्ट कमिटी ने दोनों वैक्सीन के उपयोग की मंजूरी दी है? यह चिंताजनक है कि भारत बायोटेक को तीसरे चरण का क्लीनिकल ट्रायल पूरा करने के लिए पर्याप्त संख्या में वॉलंटियर्स नहीं मिल रहे हैं और कोवैक्सीन को मंजूरी दे दी गई।”
